Dillon Family Medicine will join the federal government and other partners to support large-scale health care transformation among clinicians nationwide.
Dillon Family Medicine was selected to participate in the Transforming Clinical Practice Initiative (TCPI) launched by the Center for Medicare and Medicaid Services (CMS) and announced by Health and Human Services Secretary Sylvia M. Burwell in October 2015. The initiative aims to transition volume-driven clinical practices to value-based, patient-centered health care services by supporting more than 140,000 clinicians over the next four years in sharing, adapting and further developing their comprehensive quality improvements strategies.
Dillon Family Medicine has joined the Initiative through QualityImpact, the Consortium for Southeastern Hypertension Control (COSEHC) practice transformation network. QualityImpact is one of 29 practice transformation networks receiving TCPI funding to help equip clinicians with technical support and assistance to improve quality of care, increase patients’ access to information, and spend health care dollars more wisely.

“Our PTN will provide Dillon Family Medicine with the technology, processes, and tools to improve overall care delivery with an emphasis on putting the patient first.”
In working with QualityImpact, Dillon Family Medicine will benefit from clinical quality improvement expertise, workflow optimization consulting services, population health analytics, and the opportunity to engage and work with other selected clinicians to achieve the common goal of improved care, better health and reduced cost.
